School, Sports, and Camp Phyisicals
Liberty Urgent Care is proud to care for the children in our community!
When your child needs a school, sports, and or camp physical, please consider allowing us the opportunity to provide that service.
Our professional medical experts will review your child’s medical history, perform a comprehensive physical exam, and provide useful information on injury prevention. We can also offer guidance in the event of a suspected concussion, broken bone, heat exhaustion, fainting, asthma, and/or medical issues that may be hereditary to your family.
In the event there is clinical evidence that suggests concern, the parent{s} will be notified immediately and an appropriate medical solution will be offered. Should paperwork need to be completed to allow participation in an activity, we will gladly do so at the time of your visit.
What to Expect
During the physical exam, the provider will check your child’s:
- height, weight, blood pressure, eyes, ears, throat, abdomen, heart and lungs
What to Bring
- Participation Forms (as needed)
- If your child wears glasses or contacts, bring those.
